	In 2011, Freedom Health completed the NCQA-Certified HEDIS ^® Compliance Audit™ and received the 2011HEDIS Compliance Audit Seal for successfully achieving quality improvement and a higher level of integrity.
	SCGA - Senior Choice Gold Award for the last 5 years! Freedom Health, a Florida Health Insurance plan, has been nationally recognized for its overall value in the design of its medical and Part D prescription drug benefits! The Senior Choice Gold Award is based on estimated cost sharing comparison conducted in over 120 cities by HealthMetrix Research Inc, an independent research company that annually compares the real costs of Medicare health insurance plans to consumers. The study identifies the Medicare Plans with the lowest and highest annual out-of-pocket costs based on approved member copayments, deductibles and premiums that appear in the Medicare Options Compare database.

Freedom Health is a health Plan with a Medicare contract. Medicare approved MAPD HMO plans available to anyone entitled to Part A and enrolled in Part B of Medicare through age or disability (for MA plans, individuals must have both Part A and Part B). Medicare approved HMO Special Needs Plans (SNPs) available to anyone who meets the specific eligibility requirements of the SNP and is enrolled in both Part A and Part B of Medicare through age or disability. (To qualify for a Chronic Disease SNP, physician diagnosis of the disease must be verified prior to confirmation of enrollment. People who do not have the condition will be disenrolled. To qualify for a Dual Eligible SNP (DSNP), you must also be eligible for Medicaid assistance from the State. Premium for the DSP and copayments/co-insurance for Low Income Subsidy eligible beneficiaries may vary based on income. Enrollment period restrictions apply. Call the plan for details. You must continue to pay your Medicare applicable premiums if not otherwise paid for under Medicaid or by another third-party. Plans may be renewed annually. All plan types may not be available in all areas. Copayment and authorization rules may apply. We are required to inform you by law, that we can choose not to renew our contract with CMS, reduce our service area or no longer offer services in the area where you reside. CMS may also refuse to renew our contract, thus resulting in a termination or non-renewal. Should this occur, this may result in termination of your enrollment in the plan.
