Medicare Advantage Part C Plans

Medicare Advantage Plans work like traditional HMOs and PPOs. You may already be familiar with these types of plans. We also offer Special Needs plans for those who qualify with a chronic illness as well as both Medicare and Medicaid. All our Medicare Advantage Plans come with preventive care benefits to help keep you healthy and on the go.

With Freedom Health’s HMOs, you benefit from our leading network of HMO doctors and care providers. We offer plans with monthly premiums as low as $0, with low co-pay doctor visits as well. On an HMO plan, you’ll choose a primary care physician (PCP) who you’ll see for all your check-ups and regular exams. Your PCP also directs you to the right specialists when you need them.

Special Needs Plan

Freedom Health’s Dual Eligible & Chronic Special Needs Plans (DSNP/CSNP) are specifically designed for people with both Medicare and Medicaid or a chronic illness (respectively). Special Needs Plans are there to help you manage your health or health conditions. They’re HMO plans with prescription drug coverage that’s included. So, you’ll get Medicare coverage and prescription drug coverage, plus lots of additional benefits to help keep you healthy. Most plans have premiums for $0 with no medical copays or deductibles.

PFFS plans let you see any doctor who accepts the plans payment. However, it is important to note that not all doctors accept all PFFS Plans.

PPO

Preferred Provider Organization

With PPO plans, you’ll still pick a primary care physician for check-ups, but you can also choose to see any doctor or specialist in your network without needing a referral. PPO’s let you see any doctor, but you pay less if you see plan-approved doc

Freedom Health does not currently offer PPO plans.

Point of Service (POS) plans cover some services whether they are performed by in-network or out-of-network providers. You may pay less if you use in-network doctors, hospitals, and other healthcare providers. Referrals may still be required by your PCP to see specialists.

Medicare Advantage Plans: Part C

Medicare Part C is also called Medicare Advantage (MA). Medicare Advantage Plans include all the benefits of Parts A and B, which cover hospital and medical costs. Most MA plans also include Medicare Part D (prescription drug coverage). These plans combine health insurance and prescription drug coverage in one convenient and low-cost plan.
